RD21 remediation and requalification

SQLite repair implemented; formal RD21 UNRESOLVED, Phase 9 NO-GO, Phase 10 NO-GO, production NO-GO. Required MariaDB runtime qualification is unavailable on this host.

The dated acceptance addendum is the current assessment. The original Phase 9 decision and all original evidence remain unchanged historical records.

Product qualification ends at commit 1b89a77 on fix/rd21-acknowledged-edit-loss, based on 8e7e3f6387544e24082ebe2c107c0b868897959e. Later reporting commits do not change qualified product inputs. The final delivery message supplies the reporting commit and direct remote comparison, avoiding a self-referential commit hash in this document.

Machine-readable results retain case identities, process outcomes and original raw hashes. Raw test outputs, traces, synthetic databases and generated PDFs remain in ignored .artifacts/rd21/; portable observations and oracle results are committed here. This does not make raw artifacts available from a fresh clone or claim independent human/device/provider verification.
